In today's environment every effort must be made to hold down costs if we are to continue to provide high quality health care.   For your convenience, most major health insurances are accepted by us.

Monthly billing and open accounts are time consuming and costly. Please help by paying office fees, co-payments and deductibles upon completion of your visit.
For your convenience we accept cash, personal checks, Visa, and MasterCard.

You will be given an itemized receipt suitable for insurance purposes. Other payment arrangements can be made with our billing specialists.  If costly procedures or extended treatments are required, we will be happy to bill your insurance.

Since we are specialists, all patients with managed care insurance that require a referral must make sure that their primary care physician has supplied our office with a referral form for each visit or procedure. This is the patient's responsibility.

Please bring your insurance identification card with you to the office so that we may make a copy for your chart.
